All WISD students need:
- Utility bill or proof of residence in their school zone
- Payment for all bills owed from the previous school year (textbooks,
library fines)
- Immunization records
New students additionally need:
- Birth certificate and social security card
- Legal papers (divorce/separation)
IMMUNIZATIONS:
Public school children in Texas are required to have been vaccinated for eleven communicable diseases before entering grades kindergarten through twelfth-grade. Required immunizations are listed below. You can also click here for a printer friendly version of required immunizations.

- Diphtheria, Tetanus and Pertussis - Kindergarten through sixth-grade students must have received four doses of the diphtheria, tetanus, and pertussis vaccine including one dose on or after the fourth birthday. Seventh-graders must have received three doses of the vaccine containing diphtheria and tetanus including one dose on or after the fourth birthday. A tetanus booster is required for seventh-graders if five years have passed since the last dose. A tetanus booster is required for eighth through twelve-graders if ten years have passed since the last dose.
- Polio - All students must have three doses of polio vaccine and one dose must have been received on or after the fourth birthday. Or, a student may have had four doses of polio vaccine (any combination of IPV and OPV) before the fourth birthday.
- Measles, Mumps and Rubella - All students must have two doses of a vaccine containing measles and one dose must have been received on or after the first birthday. All students must have one dose of a vaccine containing rubella and it must have been received on or after the first birthday. All students must have one dose of a vaccine containing mumps and it must have been received on or after the first birthday. (Serologic evidence
of immunity or infection is acceptable in lieu of vaccine)
- Hepatitis A - Incoming kindergarten students must have two doses of a vaccine containing hepatitis A. (New for 2009-2010)
- Hepatitis B - All students must have three doses of a vaccine containing hepatitis B.
- Meningococcal - Seventh-grade students must have received one dose of the meningococcal vaccine. (New for 2009-2010)
- Varicella - All students must have two doses of a vaccine containing varicella and it must have been received on or after the first birthday. (Serologic evidence of immunity
or infection is acceptable in lieu of vaccine)
