All WISD students need:
- Utility bill or proof of residence in their school zone
- Payment for all bills owed from the previous school year (textbooks,
library fines)
- Immunization records
New students additionally need:
- Birth certificate and social security card
- Legal papers (divorce/separation)
IMMUNIZATIONS:
Public school children in Texas are required to have been vaccinated for eleven communicable diseases before entering grades kindergarten through twelfth-grade. Required immunizations are listed below. For your convenience, you may also choose to download printer friendly versions here.

- Diphtheria, Tetanus and Pertussis - Kindergarten through sixth-grade students are required to receive at least four doses of the diphtheria, tetanus, and pertussis vaccine with the fourth dose received on or after the fourth birthday. Seventh-graders through seniors are required to receive three doses of the vaccine containing diphtheria and tetanus with one dose received on or after the fourth birthday. A tetanus booster is required for seventh-graders if five years have passed since the last dose. A tetanus booster is required for seventh-graders if five years have passed since the last dose. A tetanus booster is required for eighth-graders through seniors if ten years have passed since the last dose.
- Polio - All students are required to receive at least three doses of a polio vaccine with one dose received on or after the fourth birthday.
- Measles, Mumps and Rubella - Kindergarten and first-graders are required to receive two doses of MMR with one dose received on or after the first birthday. Second-graders through seniors are required to receive two doses of the measles vaccine and one dose each of the rubella and mumps vaccine. (Serologic evidence
of immunity or infection is acceptable in lieu of vaccine)
- Hepatitis A - Incoming kindergarten students are required to receive two doses of the hepatitis A vaccine.
- Hepatitis B - All students are required to receive three doses of the hepatitis B vaccine.
- Meningococcal - Seventh-grade and eighth-grade students are required to receive one dose of the meningococcal vaccine.
- Varicella - Two doses of varicella are required for kindergarten, first-grade, seventh-grade and eighth-grade students. One dose is required for all other grade levels. The first dose of varicella must be received on or after the first birthday. (Serologic evidence of immunity
or infection is acceptable in lieu of vaccine)
